Who can join Total Community Care?

We serve individuals who are:
  • At least 55 years old
  • Living within specific zip codes
  • Eligible for Medicaid or able to pay privately
  • In need of nursing facility level of care
  • Able to live safely in the community


How do I join Total Community Care?

Call us at (505) 924-2650 and ask to speak to Intake. An intake coordinator will arrange for you to visit a Total Community Care Day/Health Center and will talk to you about your needs and the program.

What happens to my Medicare/Medicaid coverage when I join?

Participants with Medicare and/or Medicaid authorize Total Community Care to provide services to you. This means that you maintain benefits covered by Medicare and/or Medicaid and become eligible for additional benefits offered by Total Community Care. Individuals who are currently enrolled in an HMO will disenroll in order to join Total Community Care.


Does anyone live at Total Community Care?

Our participants live in their own homes, with family members, in assisted living, or in adult family homes.


Can I join without going to the day health program?

Attending the Day/Health program is optional, however there may be times when individuals must come to the center to be seen in the clinic. Regular attendance is highly encouraged since the Total Community Care staff would like to carefully monitor your health and safety.


Do I have to change doctors?

Yes. However, people who join Total Community Care enjoy personal relationships with our doctors and nurses and, in fact, notice that they have more access to health-care providers than they did before enrolling in the program. Photos and descriptions of our providers are on this Web site. We help make the transition easy.


What happens if I need to see a specialist?

If you need to see a specialist, such as the cardiologist or the pulmonologist, we will make the arrangements and provide the transportation.


What happens if I am no longer able to stay in my home?

Total Community Care will do everything possible to keep you in your own home. However, if you are no longer able to remain safely in your home, Total Community Care will look for a facility such as assisted living or a nursing home to meet your needs. You will still be in Total Community Care and you continue to come to the center on a regular schedule so that you can receive your medical care, therapies, and other services.


What happens if I join Total Community Care and I don’t like it?

You may disenroll from the program at any time. A Total Community Care Social Worker will assist you.


What happens if I am too sick to go to the Day/Health Center?

If you are feeling too sick to attend the center, call your center and ask to speak to a nurse. Depending on the situation, a provider may come out to see you or we will help you come to the center to be seen in the clinic. Our van will pick you up.


How much does it cost to join?

If an individual is... Then...
Eligible for Medicaid Medicaid pays for the program (participant pays nothing)
Eligible for Medicare but not Medicaid Participant pays the Medicaid portion of the program
Medicare and Medicaid eligible Participant pays nothing
Neither Medicaid nor Medicare eligible Participant pays the private pay rate. Ask for you intake worker about this amount

ELIGIBILITY

In order to qualify for the Total Community Care program, individuals must:
  • Be at least age 55
  • Be able to safely live in a community setting
  • Have specific long-term care needs
  • Live within the area shown on the map
  • Zip Codes served by TCC
