Total Community Care, New Mexicos First PACE Provider is a unique program of care for individuals 55 years of age and older. At PACE, we provide and coordinate care for elders so that they can remain as independent as possible and in their own home and communities. The PACE program includes personal care, medical services, physical therapies, day programs, prescriptions, meals and transportation. With our care, its like adding a whole team of professionals to your extended family.
Total Community Care has been in the Albuquerque community for a number of years as a pre-PACE provider, sponsored by St. Joseph Senior Care. Total Community Care was awarded full PACE provider status in June 2004 and is now operates independently as a non-profit LLC.
PACE is a national program that provides health care and in-home services for frail elderly individuals in an effort to keep them as independent as possible and out of nursing homes. PACE programs receive reimbursement from both Medicaid and Medicare. PACE, as its name implies, is all-inclusive bringing its participants a full range of services that include: primary, specialty and emergency medical care; day center programs; transportation; prescriptions; meals; physical and occupational therapies; in-home care and assistance. Each persons range of services is determined by need. Participants do not need appointments for the doctors or nurses and can be seen every day, if necessary. The program also includes supports groups and education for family caregivers.
Once a person enrolls in Total Communtiy Care, all services including medical care are provided through TCC. The participant or family must pay for any resources used from outside the service system.
For participants who are eligible for Medicare, TCC will provide all Medicare-covered services.
We accept both Medicaid or a flat monthly rate. |
